Since Community Medical and Dental Care opened, our patients have depended on us to provide top quality Primary Care.

Part of our mission is keeping our patients healthy.  This includes performing regular health screenings as well as keeping annual immunizations up to date.  As a pillar of the medical community, we offer comprehensive treatment for a full range of ailments, diagnostic testing and integration with specialists where needed.  From age 22 and up our primary care professionals deliver personalized and customized care to all of our patients..

PRIMARY CARE TEAM

JAMES ISRAEL, MD

Dr. James Israel is the longest-employed physician with CMADC, having joined us when our clinic opened in 1993. He completed his medical education at SUNY Downstate Medical Center in 1970; his Residency at Maimonides Medical Center in 1973; and his Fellowship in Gastroenterology at the Albert Einstein College of Medicine in 1975. He continues to be one of our most popular physicians, due to his empathetic bedside manner and the time he takes with each patient.

DEALTRE LOLO, MD

Hailing from Haiti, where he completed his medical education and training from the Faculty of Medicine of the State University of Haiti in 1991, Dr. Lolo migrated to the United States and took on a position at Metropolitan Hospital as a Hospitalist, and currently holds positions as an Assistant Clinical Professor of Medicine and as Chairman of the hospital’s Utilization Review Committee. Dr. Lolo joined CMADC in 2007.

BRIAN BLITZ, PA-C

Brian Blitz received his medical training from Long Island University, The Brooklyn Hospital in conjunction with Downstate Medical School. For several years he worked in community health, primary care, and emergency medicine settings. In addition, Brian was employed at Beth Israel Medical Center, NYU Hospital for Joint Diseases, and Charter Behavioral Hospital for a number of years, before joining CMADC in 2009. Brian Blitz has extensive experience in medical publishing and professional medical education initiatives including work with the McGraw-Hill Companies and CMP Media. He is a Past Vice President of the New York State Society of Physician Assistants, Adjunct Clinical Assistant Professor at the Long Island University PA Program and a board certified and re-certified Physician Assistant.

LAWRENCE ROSS, MD

YUWANNA SAVARY-LANDAU MSN, ANP-C

Yuwanna Savary-Landau MSN, ANP-C is a Board Certified Adult Nurse Practitioner, specializing in Internal Medicine.  She has been involved in medicine for over 40 years.

Yuwanna completed her Master of Science in Nursing at Molloy College School of Nursing in 1998.  She is also licensed in Critical Care, Palliative Care and as an HIV specialist.

Her area of expertise include: Preventative care; pain management; infectious diseases and end of life care.

ELIANA ROSENFELD, PA

Eliana Rosenfeld P.A. graduated Magna Cum Laude from Touro College School of Health Sciences as a Physician Assistant.  Eliana has provided services in emergency medicine, primary care, behavioral health and family medicine.  Eliana practiced medicine at Maimonides Hospital, Department of Cardiology in Electrophysiology.  Many adult and elderly patients have benefited before and after surgery from Eliana’s services. In addition, she worked for several years with a diverse population of children and adults with significant developmental disabilities in two NYC CARES clinics.  Currently, at Monsey Health Center, Eliana divides her time between Pediatrics and Adult Medicine.  She diagnoses and treats adult patients with acute and chronic conditions such Hypertension, Diabetes and Cardiovascular disease.  In addition, Eliana diagnoses and treats our pediatric patients who are ill as well as providing preventative and wellness services.
