Heather Cook, MD
Dr. Heather (Chana) Cook received her medical degree from the Sackler School of Medicine at Tel Aviv University in 1994. Previously, she attended Columbia University’s post baccalaureate pre-med program, graduating in 1990. She completed her internship and residency at New York University’s Pediatrics Program in 1997. Her pediatrics experience includes DOCS Medical Practice of Yonkers, where she performed well child checkups, sick visits, acute and chronic asthma, and administered vaccines. She was also a general pediatrician at DOCS Medical Practice of the Bronx. Dr. Cook is affiliated with Mount Sinai Medical Center and has been with CMADC since 2015.

Abraham D. Leiser, FNP
Abraham Dovid Leiser, FNP comes to Monsey Health Center with over 20 years of experience in the medical field. He is a certified EMT and earned his degree in Nursing from Touro College in Brooklyn, NY. He then went on to complete his certification as a Family Nurse Practitioner at United States University’s MSN-FNP Program.
In addition to his vast experience, Abraham applies compassion, understanding and sensitivity when dealing with his young patients.

Eliana Rosenfeld, PA
Eliana Rosenfeld P.A. graduated Magna Cum Laude from Touro College School of Health Sciences as a Physician Assistant. Eliana has provided services in emergency medicine, primary care, behavioral health and family medicine. Eliana practiced medicine at Maimonides Hospital, Department of Cardiology in Electrophysiology. Many adult and elderly patients have benefited before and after surgery from Eliana’s services. In addition, she worked for several years with a diverse population of children and adults with significant developmental disabilities in two NYC CARES clinics. Currently, at Monsey Health Center, Eliana diagnoses and treats adult patients with acute and chronic conditions such Hypertension, Diabetes and Cardiovascular disease. In addition, Eliana diagnoses and treats our pediatric patients who are ill as well as providing preventative and wellness services.
